Qualifications

• Associate degree in health or social services or 2 years related experience.
• Clearance through the Idaho Department of Health and Welfare Criminal History background check.
• Demonstrated willingness to work with low-income and unhoused populations.
• Understanding of barriers for people with behavioral health and substance misuse problems.
• Working knowledge of Boise area social service and housing resources.
• Ability to work as a team member.
• Good verbal and written communication skills.
• Reliable and courteous.

Responsabilités

• Identify new and existing patients to determine program eligibility by reviewing registration information, screenings, and income.
• Maintain a caseload of eligible participants and work to eliminate barriers to housing stability; meetings may occur in clinic, community, or at patients' homes.
• Complete a housing stability plan with each participant.
• Assist participants through each step of the housing process (search, application, move-in, ongoing support).
• Advocate for participants with authorized releases as needed.
• Maintain linkages with other programs and local housing resources.
• Assist participants with paperwork for securing and maintaining housing.
• Document encounters in a timely manner per SOPs.
• Facilitate referrals to clinic staff and other services as needed.
• Serve as a housing resource for clinic staff.
