Qualifications

• Three years post-secondary education or three years' experience in lieu of education.
• Experience working with underserved populations.
• Knowledge of local social service resources for behavioral health issues.
• Ability to work independently.
• Excellent communication and customer service skills.

Responsibilities

• Provides housing services expertise and ensures adherence to program policies and procedures.
• Coordinates supportive services including applications, outreach, home visits, referrals, crisis intervention, case management and advocacy.
• Leads and supports staff, conducts needs assessments, and develops service plans for participants.
• Maintains linkages with health care providers and social services to support participants.
• Coordinates housing referrals and supports timely monthly reporting to funders.
