Job summary

LA Family Housing seeks an Engagement Coordinator to assist participants in accessing services for housing crisis. You will perform triage, intake, eligibility screening, and case management, maintaining records in HMIS/CHAMP across service centers and in the field.

You will conduct housing plans, coordinate with LAFH teams and community partners, and ensure accurate documentation for program reports, while embracing trauma-informed, harm-reduction practices and HIPAA standards.

Qualifications

• 1+ year of relevant work history.
• Experience with trauma-informed and harm-reduction approaches.
• Ability to pass background checks and HIPAA requirements.

Responsibilities

• Conduct triage, screening and assessments onsite or field-based.
• Develop housing plans with participants.
• Provide housing-focused case management.
• Conduct risk and safety assessments and crisis interventions.
• Respond to referrals and walk-in appointments daily.
• Coordinate referrals with LAFH and community partners.
• Maintain participant records and eligibility documents.
• Data entry into HMIS/CHAMP for monthly/quarterly/annual reports.
