Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform in‑home health assessments for Medicare Advantage and related plans. This field‑based role focuses on education, gap closure, and collaboration with primary care providers to improve health outcomes.

The position offers a flexible schedule with 8am–8pm coverage across a 7‑day week, no on‑call, weekends, or holidays. Compensation ranges from $109,500 to $164,000 plus a comprehensive benefits package.

Qualifications

• Active ANCC or AANP certification for NP, or NCCPA certification for PA.
• Active, unrestricted state licensure where practice will occur.
• Driver's license and reliable transportation for home visits.
• Ability to obtain licensure in start state if required.
• Ability to operate in varied home environments.

Responsibilities

• Conduct comprehensive in‑home assessments for Medicare Advantage and other plans.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and arrange referrals.
