Job summary

UnitedHealth Group's Optum Home & Community Care is seeking a Nurse Practitioner - House Calls to perform in-home health assessments across Georgia counties. You will review medical history, conduct exams, and coordinate with primary care providers to close care gaps.

Flexible provider schedules available between 20-40 hours per week with no on-call, weekends, or holidays. Must have ANCC/AANP certification and an active GA license; fluency in other languages is valued.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted license to practice in the state (GA) with no disciplinary actions.
• 1+ years of clinical home health or similar setting preferred.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plan members.
• Review past medical history, meds, vitals, and perform exams.
• Educate patients and coordinate with PCPs to close care gaps.
• Identify urgent situations and provide appropriate interventions.
• Document findings and support care management and planning.
