Job summary

Community Options, Inc. in Cookeville, TN seeks a Foster Care/Host Home Provider to offer a safe, supportive home for individuals with developmental and/or physical disabilities. This independently contracted role creates a family-like setting with ongoing supervision by the organization.

You must have a HS diploma, a valid driver’s license, and prior DSP experience, plus willingness to complete Community Options trainings and allow home visits.

Qualifications

• High School Diploma or GED required.
• Valid Driver's License required.
• Previous Direct Support Professional experience required.
• Must complete trainings through Community Options.
• Must allow supervisory visits in your home.

Responsibilities

• Provide a safe, comfortable home environment for residents.
• Build supportive, genuine relationships with individuals served.
• Coordinate with supervisors and complete required trainings.

This independently contracted position will provide a 24/7 family-like setting for an individual.

We are looking for someone in the Cookeville, TN area!

We are seeking someone who is available to provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high-quality life. You must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support.

REQUIREMENTS:

• High School Diploma/GED
• Valid Driver's License
• Previous experience working as a Direct Support Professional is required
• Will need to successfully complete trainings through Community Options
• Must be open to supervisory visits in your home

• Our Host Home Providers are contracted and reimbursed at a per-diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
