Job summary

Community Options, Inc. in Chattanooga, TN is seeking Home Family Model Providers to open their homes to individuals with developmental and/or physical disabilities, offering a safe and supportive 24/7 family-like setting.

The position is independently contracted, with compensation consisting of a per diem rate plus monthly room and board provided by the individual being supported. Training through Community Options is required, and supervisory visits to your home may occur.

Qualifications

• High School Diploma or GED required.
• Valid driver's license required.
• Previous experience as a Direct Support Professional is required.
• Must successfully complete trainings through Community Options.
• Must be open to supervisory visits in your home.

Responsibilities

• Provide a safe, comfortable home and meet the basic physical and emotional needs of the individuals served.
• Establish a genuine, supportive relationship with the person you house and support.
• Offer a 24/7 family-like setting as an independently contracted provider.

We are seeking someone in the Chattanooga, TN area, who is available to provide a safe and comfortable home, while ensuring the basic physical and emotional needs required for our individuals to live a high-quality life. You must have the ability to establish a comfortable, supportive, and genuine relationship with the individual you house and support. This independently contracted position will provide a 24/7 family-like setting for an individual.

REQUIREMENTS:

• High School Diploma/GED
• Valid Driver's License
• Previous experience working as a Direct Support Professional is required
• Will need to successfully complete trainings through Community Options
• Must be open to supervisory visits in your home

Our Family Model Providers are contracted and reimbursed at a per diem rate PLUS they receive monthly room and board from the individual they support – this income is tax exempt.
