00 RHA Health Services, LLC in Georgia seeks caring individuals to become Host Home Providers. This Independent Contractor role supports adults and children with I/DD in a home setting, providing training and supervision through RHA's team while you deliver person-centered care.

Formación

• HS Diploma/GED preferred
• 18 years old or older
• Documentation of home ownership: mortgage or lease
• Proof of homeowner's or renter's insurance
• Complete all Host Home Study requirements including training
• Valid Driver's License; reliable transportation with valid vehicle insurance
• Ability to pass criminal background and registry checks as well as a drug screen
• Work as an Independent Contractor

Responsabilidades

• Understanding the individual in the context of their history, networks, and hopes for the future.
• Support with meals, rest, daily self-care, leisure, education and work.
• Prepare, teach, and assist with food preparation and home cleaning.
• Honor the choices, preferences and culture of the person supported.
• Provide opportunities for social, emotional and spiritual needs.
• Assist with transportation and scheduling medical, therapy, vocational and recreational activities.
• Ensure a safe and well-maintained home environment.
• Implement services addressing goals identified in each Person Centered Plan.
• Attend treatment team meetings and communicate about individual needs and progress.
• Participate in supervision as agreed with the RHA supervisor.
• Complete daily documentation for billing in the EHR.
• Maintain training compliance such as CPR and Safety Care.
• Maintain equipment (Laptop, phone, etc.).
• RHA will assist with crisis training, respite care, and supervisor support.
