Job summary

RHA Health Services, LLC invites caring individuals to join as Family Model caregivers in Tennessee. You will support adults or children with I/DD in their home, following the Person Centered Plan and providing respectful, person-centered care.

As an Independent Contractor, you receive training and supervision from RHA. Compensation is determined per contract, with focus on home-based, community-integrated supports for individuals in need.

Qualifications

• HS Diploma or GED required.
• 18 years old or older.
• Valid Driver's License and reliable transportation.
• Ability to pass criminal background and registry checks.
• TB screenings for all household members.
• Independent Contractor status.

Responsibilities

• Support individuals with disabilities in a home setting according to needs.
• Assist with daily living activities, meals, rest, personal care and mobility.
• Honor choices, preferences and culture of the person supported.
• Coordinate transportation and scheduling of medical, therapy and activities.
• Attend supervision and follow the Person Centered Plan with care.
