Responsibilities

• Coordinate and deliver primary and nursing care in collaboration with the Practice Site Physician.
• Maintain and review patient records and test results.
• Triaging calls and providing consultation; manage medical emergencies as appropriate.
• Support education and follow-up programs to measure client outcomes.
