• 3+ years of hospital-based nursing experience
• Experience in discharge planning and transitions of care
• Strong communication and relationship-building skills
• Ability to collaborate across multiple departments and disciplines
• Knowledge of Epic EMR or similar EMR systems
• Nursing licensure in NC (required)
