Job summary

Patient Engagement Advisors (PEA) is seeking a full-time Transition Specialist to work onsite at AdventHealth Ocala. You will help patients transition from hospital to the next stage of care, coordinating transportation, delivering discharge medications, and connecting to community resources.

The role emphasizes face-to-face patient interaction, teamwork within the clinical care setting, and a clear career path through PEA's progression program.

Qualifications

• High School Education or equivalent is required.
• 6 months pharmacy technician or 1-year healthcare-related experience required.
• Other related healthcare or medical experience will be considered.

Responsibilities

• Schedule patient’s transportation as needed.
• Deliver discharge prescription medications to patient’s bedside.
• Assist patients with medication affordability.
• Coordinate home meal delivery service for patients.
• Connect patients to community resources as needed.

Patient Engagement Advisors (PEA)is seeking a full timeTransition Specialistto work onsite at AdventHealth Ocala.

Schedule: Monday - Friday 9:00 AM - 5:30 PM with weekend rotation 10:30 AM-7:00 PM as needed,onsite at AdventHealth Ocala

Job Details:

As a Transition Specialist, your role is to set patients up for success to transition out of the hospital into their next stage of care.

Here are some of the tasks you will perform to assist in this transition:

• Schedule patient’s transportation as needed
• Deliver discharge prescription medications to patient’s bedside
• Assist patients with medication affordability
• Coordinate home meal delivery service for patients
• Connect patients to community resources as needed

Requirements:

• High School Education or equivalent required
• 6 months pharmacy technician (retail preferred) OR 1-year medical assistant / patient care technician / patient care coordinator / clinic front desk associate / nursing assistant / home health aide / other healthcare or medical experience required
• Other related healthcare, hospital, or medical experience will be considered
• Ability to report to work onsite as scheduled; ability to be on feet – standing, walking, and/or climbing stairs 8-10 hours daily
• Ability to receive required vaccinations (as required by PEA or hospital) unless approved for exemption

Salary Range: $18-19/hour
