Job summary

Patient Engagement Advisors (PEA) seeks a full-time Transition Specialist to work onsite at AdventHealth Heart of Florida. The role includes duties as an alternate Logistics Transition Specialist for local courier tasks and medication delivery.

You will educate patients on discharge requirements, arrange follow-up care, and connect patients to resources while ensuring timely handoffs to home or hospital locations. Requires on-site presence and patient-focused service.

Qualifications

• High school education or equivalent required.
• Valid driver’s license with clean driving record.
• 6 months pharmacy technician or 1 year healthcare experience required.
• Retail pharmacy experience strongly preferred.
• Ability to report onsite as scheduled; able to stand/walk 8–10 hours.

Responsibilities

• Deliver medications and supplies to patients’ homes or hospital locations for hand-off.
• Schedule follow-up appointments and transportation as needed.
• Assist patients with medication affordability and access.
• Coordinate home meal delivery and connect to community resources.
• Document social determinants of health information for each patient served.
• Utilize PEA software and programs to optimize savings for patients.
