Job summary

Qualifications

• Master's degree of Social Work from an accredited training program.
• LICSW in the District of Columbia is required.
• Minimum 2 years experience with acute and chronically ill patients; hospital setting preferred.
• Experience with oncology patients preferred.

Responsibilities

• Provides clinical social work services to identified patients and families in inpatient and outpatient settings, including individual and group counseling.
• Collaborates with a multi-disciplinary team and documents in medical records; facilitates communication among care teams.
• Coordinates links between patient, family, hospital and community resources to assure continuity of care.
• Represents social work perspective and serves on hospital and Lombardi Cancer Center committees and community organizations.
