Job summary

Qualifications

• Master’s degree in Social Work from an accredited program.
• 2 years' experience with acute and chronically ill patients (Hospital setting preferable).
• LICSW (Licensed Independent Clinical Social Worker) license.

Responsibilities

• Provides clinical social work services to identified patients and families in inpatient and outpatient settings, including individual and group counseling.
• Coordinates care between patient, family, hospital, and community resources to ensure continuity of care.
• Participates in orientation, training, and education programs for students, physicians, nurses, and other health care professionals; may provide supervision to junior social workers.
