Job summary

CHRISTUS Health is seeking a hospital social worker to support patients and families across the continuum of care. You will interview, assess, and coordinate psychosocial care with the interdisciplinary team to meet age-specific needs and improve outcomes.

The role emphasizes discharge planning for complex patients, linkage to community resources, advocacy for patient autonomy, and collaboration with physicians, nurses, and social service staff. Work schedule typical 8AM–5PM, full-time.

Qualifications

• Master's Degree in Social Work from accredited institution.
• Strong interview, assessment, organizational and problem-solving skills
• Excellent interpersonal communication and negotiation skills
• Ability to work with people of all social, economic and cultural backgrounds
• Ability to analyze and manage change
• Ability to work independently and to develop relationships with physicians, families, patients, interdisciplinary team and community agencies
• Demonstrates ability to connect patients and families with necessary services
• Maintain working knowledge of resources and services available in the community including options for those with limited or non-existent funding options

Responsibilities

• Uses psychosocial assessment to guide care and develop an interdisciplinary plan.
• Intervenes with emotional, social, and financial consequences of illness; mobilize resources.
• Advocates for patient autonomy and access to services within the health care system.
• Assists with discharge planning for complex patients and post-acute care referrals.
• Provides brief counseling and intervention services as needed.
• Collaborates with RN Case Managers, physicians, and the treatment team on discharge status.
• Initiates referrals to post-acute care providers and community services.
