Responsibilities

• Provides psychosocial assessment and intervention and facilitates referrals to community resources.
• Develops individualized plans for discharge and continuity of care.
• Communicates with physicians, families and the multidisciplinary team to ensure patient self-determination.
• Maintains knowledge of community resources and documents care plans in the medical record.
• Assists with crisis intervention and reports abuse as required by law.
