Job summary

Advocate Health is seeking a medical social worker to assess post-hospital care needs and support patients and families. You will partner with the Clinical Care Management team to evaluate appropriate levels of care and coordinate referrals for community resources.

Strong communication is essential to support patients during transition from hospital to next care setting. Required MSW; LCSW licensure preferred.

Qualifications

• Masters in Social Work required.
• LCSW licensure preferred.
• At least 1 year professional hospital/health setting experience preferred.

Responsibilities

• Complete psychosocial assessments for post-hospital care.
• Assist patients in coping with stress related to hospitalization, disability, chronic/terminal illness.
• Collaborate with Clinical Care Management to determine appropriate level of care.
• Serve as a resource on emotional, social, and psychosocial aspects of illness for staff and physicians.
