Job summary

MultiCare is seeking a Hospital Social Worker for cross continuum care coordination, psychosocial assessments, and interventions to improve patient care and population health. You will identify opportunities to enhance services and coordinate care across multiple settings.

The role requires a Master’s in Social Work, licensure in Washington (LICSW/LSWAIC with LICSW planned within four years), and at least three years of hospital experience.

Qualifications

• Master’s degree in Social Work required.
• Licensed in the state of Washington or LICSW/LSWAIC with LICSW target.
• Minimum three years hospital or equivalent experience; healthcare knowledge preferred.

Responsibilities

• Perform cross continuum care activities across multiple sites as appropriate.
• Provide customer service to patient, family, physician/APP and care team, including discharge milestones.
• Model and maintain professional communications with colleagues and staff.
• Delegate responsibilities while remaining accountable for outcomes.
• Ensure medically appropriate transitions of care for patient and family.
