Job summary

Qualifications

• Licensed in the state of Washington; LICSW or LSWAIC with LICSW within four years preferred.
• Minimum three years Hospital or equivalent experience; health industry knowledge preferred.

Responsibilities

• Perform cross continuum care activities across sites of care as appropriate.
• Provide customer service to patient, family, physician/APP and care team members, including discharge milestones and progression to next level of care.
• Model and orchestrate professional communications and maintain proactive communication with colleagues and staff.
• Delegate responsibilities appropriately while remaining accountable for outcomes.
• Ensure the patient and family experience medically appropriate transitions of care.
