• Provide customer service to patient, family, physician/APP and care team to support discharge milestones and care progression.
• Model and maintain professional, proactive communication with colleagues and staff.
• Delegate responsibilities appropriately but remain accountable for outcomes.
• Ensure medically appropriate transitions of care for patients and families.
