Qualifications

• Current Licensure in Social Worker (LGSW/LCSW/LICSW) in the District of Columbia.
• Master’s degree in social work.
• Attendance and punctuality – arrive on time and manage time off with notice.
• Mandatory training, testing and competencies completed on time.
• Three years’ experience in acute care or related healthcare field preferred.
• Case Management Certification preferred.
• Experience in case management and/or discharge planning strongly preferred.
• Effective oral and written communication and interpersonal skills.
• Knowledge of government and third‑party reimbursement systems and contracts.
• Strong organizational and problem‑solving skills.
• Ability to maintain confidentiality.
• Ability to adapt to multiple and changing priorities.

Responsibilities

• Assess and plan the patient’s care from admission to post‑discharge.
• Coordinate discharge planning to ensure seamless transitions.
• Utilize critical thinking and case management principles to meet patient needs.
