Job summary

The Social Worker LCSW at Health First provides psychosocial support, discharge planning, and care coordination to ensure safe transitions for patients and families.

You will assess needs, facilitate referrals, and collaborate with the care team, physicians, payers, and community resources to optimize outcomes.

Requires Florida LCSW license, MSW, CPR/BLS, and at least one year of relevant hospital social work experience; CCM/ACM preferred.

Qualifications

• Master’s of Social Work required.
• Current Florida LCSW license.
• BLS Healthcare Provider completion required on hire.
• Minimum of 1 year social work experience.

Responsibilities

• Collaboratively determines discharge goals and communicates plans to patient/family, physicians and staff.
• Provides counseling and researches placement options for substance abuse, mental health, homeless and indigent patients.
• Facilitates transfer of Baker Act and Marchman placements to mental health facilities and delivers therapeutic interventions as needed.
• Completes mandatory reporting processes and ensures compliance with regulatory standards.
• Facilitates referrals for post-discharge psychosocial and therapeutic needs.
• Mentors social work students and provides clinical supervision for social workers pursuing licensure.
