Qualifications

• Master’s degree in social work from an accredited program.
• New Jersey Social Work License required; LSW or LCSW required; LCSW strongly preferred.
• Minimum of three years of experience as a Social Worker.

Responsibilities

• Coordinate and facilitate patient care through a multidisciplinary approach.
• Discharge planning and utilization of resources.
• Collaborate with physicians, Rehab, Respiratory, DI, Lab, etc.
• Provide case management and clinical reasoning.
