Valley Health System seeks a skilled Social Worker to coordinate patient care within the Valley Hospital Health Care System across multidisciplinary teams in Paramus, NJ. You will support discharge planning, resource utilization, and complex case management for diverse patient needs.

Position SummaryTo coordinate and facilitate patient care through the Valley Hospital Health Care System as it relates to a multidisciplinary approach to patient progression, discharge planning and utilization of resources.EducationMaster’s degree in social work from an accredited program. New Jersey Social Work License required. MSW and LSW or LCSW required.
