Qualifications

• Master’s degree in social work from an accredited program.
• NJ Social Work License (LSW or LCSW) required; LCSW strongly preferred.
• Minimum three years of social work experience.

Responsibilities

• Coordinate and facilitate patient care through discharge planning and utilization of resources.
• Collaborate with physicians, Rehab, Respiratory, DI, Lab and other team members to support patient progression.
