Job summary

The Valley Hospital in Paramus, NJ is seeking an experienced Social Worker to coordinate patient care within a multidisciplinary team across the Valley Hospital Health Care System. You will support progression, discharge planning and resource utilization, ensuring safe and efficient patient transitions.

Requirements include a Master’s degree in social work, NJ social work license (MSW + LSW or LCSW), with LCSW preferred, and a minimum of three years of social work experience.

Qualifications

• Master’s degree in social work from an accredited program.
• New Jersey Social Work License required. MSW and LSW or LCSW required.
• LCSW strongly preferred.
• Minimum of three (3) years of experience as a Social Worker.

Responsibilities

• Coordinate and facilitate patient care through a multidisciplinary approach to patient progression, discharge planning and utilization of resources.
