Job summary

Pathways Health Partners seeks a Case Manager – Nurse (RN) to round on hospitalized patients, coordinate care with bedside staff and treating providers, and assess post-discharge needs. The role includes documenting assessments for post-acute care across inpatient, ER and observation units.

The ideal candidate holds an RN license in Florida (BSN preferred) with 2+ years in case management. CPR/BLS/ABLS certification at hire or within 30 days is required.

Qualifications

• RN program completed; Florida license active
• Minimum 2 years RN Case Manager experience preferred
• CPR/ABLS/CPR/BLS certified at hire or within 30 days
• BSN preferred or combination of education and experience

Responsibilities

• Rounding patients in hospital and bedside communication
• Document assessments and care coordination across providers
• Identify post-discharge needs and barriers to care
• Conduct comprehensive patient assessments in inpatient, ER and observation units
• Maintain HIPAA compliant communications and documentation
