Job summary

Intermountain Health St George Regional Hospital is seeking a Nurse Care Manager to join our ambulatory team. This full-time, dayshift role supports three or more clinics within Primary Care initiatives, educating patients and families, coordinating services, and balancing clinical quality with cost management through various communication channels.

The ideal candidate holds an RN license, BLS, and has 3 years of nursing experience, with a BSN preferred within three years.

Qualifications

• Current RN license for the state in which the nurse practices.
• BLS certification for healthcare providers.
• RNs hired or promoted into this role need to have or obtain their BSN within three years of hire or promotion.
• Three years of clinical nursing experience.

Responsibilities

• General case management across patient populations.
• Responds to physician referrals and identifies patients who meet established criteria for care management.
• Assesses family, social, cultural characteristics and communication needs.
• Utilizes care process models to manage chronic disease and provide patient education.
• Coordinates with care managers in other settings and Provides information on enabling services.
• Pre-visit planning, reminders, transitions of care and supporting personalized primary care goals.
