Hartford HealthCare, through Charlotte Hungerford Hospital in Torrington, CT, seeks a Physical Therapist to evaluate, plan, and administer medically prescribed PT treatments. The role supports patient safety and functional outcomes within a comprehensive care network.

Under the direction of Rehabilitation Services, you will perform assessments, develop care plans, document progress, and educate patients and families while collaborating with the care team to ensure high-quality, coordinated care.

• Clinical assessment and documentation skills.

• Strong communication with patients and families.

• Performs comprehensive physical, psychosocial, and safety assessment of patients.
• Develops a Plan of Care to promote optimum function and safety.
