Job summary

Hartford HealthCare seeks a Physical Therapist to evaluate and treat patients across inpatient, outpatient, home care, SNF, or assisted living environments. You will develop individualized treatment plans, perform comprehensive evaluations, and collaborate with a multidisciplinary team to promote functional independence.

Minimum CT PT licensure, BLS, and a BS in Physical Therapy are required; new grads may have a temporary CT license for up to 120 days.

Qualifications

• Graduate education required: Bachelors in Physical Therapy and CT PT licensure
• Licensed in CT: CT Physical Therapy license required
• Temp CT PT license allowed for up to 120 days for new grads; must obtain permanent CT license per CT guidelines
• BLS certification required (on hire or within 30 days)
• For Home Care: valid driver’s license and reliable transportation

Responsibilities

• Evaluate and treat physically disabled patients using PT techniques and modalities
• Develop and implement individualized treatment plans to promote function and safety
• Perform patient evaluations, establish goals, and monitor progress
• Document evaluations, care plans, goals, and patient discharge summaries
• Coordinate with Rehab Therapists, Nurses, Social Workers and other care providers for care transitions
• May work in inpatient, outpatient, home care, SNF, or assisted living settings
• Notify physician of status changes and progress
• Assist with patient discharge planning and community resource referrals
