Hartford HealthCare in Meriden, CT seeks a Physical Therapist to administer medically prescribed PT treatments. The PT evaluates function, determines treatment plans, and delivers or delegates services to maximize functional independence.

Responsibilities include evaluating motor function, setting goals, using modalities and exercises, and coordinating care with a broader rehab team. Open to new grads; CT license required or temp license at hire.

Formación

• Bachelors, Masters or Doctoral Degree in Physical Therapy from an accredited CAPTE program.
• Current CT Physical Therapy license or active temporary license at time of hire.
• Open to new graduates.
• Previous experience as a Physical Therapist preferred.

Responsabilidades

• Performs thorough examination and evaluation of patient’s level of function after physician referral.
• Establishes measurable goals with patient/family based on care protocols and critical paths.
• Implements and delegates PT interventions and clinical decision making including modalities and therapeutic exercise.
• Provides patient and caregiver education regarding treatment and precautions.
• Participates in patient transition planning and discharge planning with community resources.
• Delegates tasks and provides supervision for PT students, Rehabilitation Aides and PT Assistants.
