Job summary

Hartford HealthCare is seeking a Physical Therapist to evaluate patients, develop individualized treatment plans, and deliver therapy across inpatient, outpatient, home care, skilled nursing, or assisted living settings. Expect collaboration with a cross-functional health care team and ongoing patient progress reviews.

The role requires a Bachelor of Science in Physical Therapy, CT licensure, and BLS certification.

Qualifications

• State CT Physical Therapy license required.
• Temp CT Physical Therapy license for new grads valid up to 120 days.
• BLS certification required (upon hire or within 30 days).
• For Home Care: valid driver’s license and reliable transportation.

Responsibilities

• Administers medically prescribed Physical Therapy treatments.
• Evaluates patient’s level of function and determines treatment strategies.
• Delivers and/or delegates PT services to maximize functional independence.
• Completes documentation including evaluation, care plan, goals, progress and discharge.
• Coordinates patient care with Rehab Therapists, Nurses, Social Workers and other providers.

Education

Bachelors of Science – Physical Therapy

Master’s Degree in Physical Therapy (preferred)

Active Basic Life Support (BLS)
