Qualifications

• High school diploma or GED equivalency.
• Basic math, medical terminology, and literacy skills.
• Excellent written and verbal communication skills.
• Proficient computer skills including Microsoft Office, Google Suite, and EPIC.

Responsibilities

• Collaborate with physical therapy, nursing, and other health professionals to meet patient mobility needs.
• Provide direct patient care focused on mobility, safety, and education for patients and families.
• Document patient progress and outcomes in the medical record; notify RN of abnormal findings.
• Maintain a safe environment and use proper body mechanics during patient handling.
• Assist with transfers, ambulation, and exercise facilitation; support discharge to home settings.
