AA Healthcare is seeking a Hospital Liaison to develop and cultivate new patient prospects for admission to the facility. The role will maintain relationships with community partners, family members, and facility personnel to ensure smooth referrals and admissions.

The position focuses on visiting hospitals, following up with social workers, case managers, and families, and coordinating with intake teams to optimize admissions and expand referrals.

Qualifications

• 2–3 years of Hospital Liaison, healthcare marketing, or skilled nursing experience.
• Experience working directly with hospital social workers, case managers, and discharge planners.
• Strong understanding of hospital referrals and the skilled nursing admission process.
• Established hospital relationships are a plus.
• Strong communication, follow-up, and relationship-building skills.
• Goal-driven with a focus on growing referrals and admissions.

Responsibilities

• Visit assigned hospitals regularly and build strong referral relationships.

• Track referrals from initial contact through admission.
• Work closely with Central Intake and facility teams to coordinate admissions.
