Job summary

Infinity Healthcare Consulting is seeking a Hospital Liaison to ensure smooth admissions transitions from acute and post-acute care. You will generate referrals, build relationships with hospital discharge planners, and drive market growth by coordinating with AD Concierge and leadership.

You will manage referrals through CarePort and CRM dashboards, align initiatives with the market, and support daily admissions with strong time management and attention to detail for optimal patient experiences.

Qualifications

• GED or High School Diploma with 2 years’ experience.
• Associate degree in healthcare, business or related field with 1 year experience preferred.
• Skilled nursing experience

Responsibilities

• Stationed daily at primary account to maximize market engagement and referrals.
• Review and update CRM Dashboard for accurate reporting via Forecast and other platforms.
• Respond to all incoming referrals on Careport and other referral platforms timely, daily.
• Provide definitive answers for all incoming referrals within an hour or by end of day, if appropriate.
• Collaborate with Business Development and Operations leadership to expedite delays, barriers, and solutions in real time.
• Collaborate with AD Concierge on daily updates regarding pending referrals for Forecast.
• Collaborate with Transitional Advisors on communication with incoming referrals and admissions on CarePort.
• Timely PCC data input including patient demographics, resident representative, POA, and Guardian information.
• Weekly Reports
• Participate in Quarterly Business Development meeting to grow census and market exposure.
• Complete daily Forecast for Team review via Census call, Morning meetings and standdown.
• Overseeing Guest Relations and Concierge initiatives including patient rounding and testimonials.

• Collaborate with AD Concierge on daily updates regarding all pending referrals for Forecast.
