Job summary

DeCoach Recovery Centre in Dayton, OH is seeking a Hospital Liaison to bridge hospitals and addiction treatment, ensuring smooth transitions from hospital care into our programs.

You will build relationships with physicians, nurses, case managers, and discharge planners; coordinate timely referrals; educate hospital teams onavailable care options and collaborate with admissions and outreach teams to support patients after discharge.

Qualifications

• 2+ years in healthcare liaison, case management, or related role.
• Bachelor's degree in communications, social work, psychology, or health administration preferred.
• Excellent interpersonal and advocacy skills.
• Knowledge of addiction treatment (MAT, detox, residential, outpatient).

Responsibilities

• Build relationships with hospital staff across the Dayton area.
• Coordinate referrals from hospital to DeCoach programs.
• Educate hospital teams about care options (MAT, residential, outpatient).
• Collaborate with admissions and outreach teams.
• Follow up with patients and contacts post-discharge.
• Represent DeCoach at community events.
