The role coordinates discharge planning, maintains documentation, and collaborates with medical and nursing teams to plan care and pre-admission assessments. Travel between hospital and facility is required.

Qualifications

• Bachelor's degree in Nursing, Healthcare Administration, Social Work (preferred).
• Healthcare regulations, discharge planning, and patient care protocols knowledge expected.
• Excellent communication and interpersonal skills.
• Ability to handle sensitive information with discretion.
• Proficiency with EHRs and healthcare management software.

Responsibilities

• Coordinate patient transitions from hospital to the skilled nursing facility; review records and discharge summaries.
• Serve as primary liaison between hospital and facility; manage inquiries, updates, and care-plan alignment; document referrals and admissions to regulatory standards.
• Collaborate with facility medical and nursing teams to review incoming patient needs and prepare for arrival; facilitate pre-admission assessments and care planning.
• Address and resolve issues related to patient transfers or care continuity; advocate for patient needs and resolve barriers to effective care.
• Develop and maintain relationships with hospital staff, discharge planners, physicians, and other personnel.
• Provide education to hospital staff about the facility's services; train facility staff on communication and collaboration with hospitals.
• Participate in quality improvement initiatives related to patient transitions and care coordination; provide feedback to enhance transfer process and outcomes.
