Qualifications

• Minimum of 1 year of experience as a Hospital Liaison.
• Strong understanding of post-acute care, skilled nursing, and discharge processes.
• Excellent communication and interpersonal skills to build relationships with hospital staff, families, and leadership.
• Highly organized with the ability to manage multiple facilities and cases.
• Compassionate, professional demeanor with resident advocacy.

Responsibilities

• Conduct daily visits to hospitals where ALIYA Healthcare residents are admitted across multiple southern suburb facilities.
• Monitor the condition and progress of hospitalized residents and communicate updates to facility staff.
• Serve as primary contact for families of hospitalized residents, providing timely updates and addressing concerns.
• Collaborate with hospital discharge planners and case managers to facilitate discharge plans.
• Coordinate with ALIYA facility staff to prepare for residents' return, ensuring a seamless transition back to the facility.
• Advocate for residents' needs and preferences throughout their hospital stay.
• Maintain accurate records of all hospital visits, interactions, and follow-ups.
• Attend internal meetings with ALIYA Healthcare staff to report on hospitalized residents' statuses and address any issues or concerns
