ALIYA Healthcare seeks a dedicated Clinical Liaison to connect residents, families, and healthcare providers. You will facilitate communication, support care coordination, and ensure residents receive the services and resources they need during their stay.

As a frontline liaison in a fast-paced skilled nursing setting, you will manage multiple priorities while providing compassionate, resident-centered care and educating families about available services.

Qualifications

• Clinical background required.
• Minimum 2 years of experience in skilled nursing or long‑term care required.
• Strong interpersonal and communication skills.
• Ability to manage multiple priorities in a fast-paced healthcare environment.
• Strong organizational and problem-solving abilities.

Responsibilities

• Serve as a primary point of contact for residents, families, and healthcare providers.
• Facilitate communication between clinical teams, physicians, and support staff.

• Support smooth transitions of care and continuity between providers.
