Qualifications

• Master's degree in social work required.
• At least one year of actual work experience in a hospital setting or an ongoing plan for consultation between the social worker and a qualified social worker must be developed.
• Must have knowledge of social and physical factors that affect functional status at discharge and knowledge of community resources to meet post discharge clinical and social needs.
• Currently licensed by the Nevada State Board of Examiners for Social Workers (LMSW)

Responsibilities

• Discharge and transfer planning for patients referred to the Care Management Dept.
• Coordinate with care teams to ensure timely and appropriate discharge planning.
