Job summary

Valley Health’s Winchester Medical Center in Winchester, VA seeks a Discharge Planner II in Social Services. You will assess patients holistically, develop individualized discharge plans, coordinate with interdisciplinary teams, and navigate post-acute resources to ensure safe, timely discharges.

The role requires a Bachelor’s in social work (Master’s preferred), 2 years of medical social work experience, and BLS certification.

Qualifications

• Bachelor of Social Work degree required; Master’s preferred for advancement.
• Minimum 2 years of social work experience in a medical setting.
• BLS Healthcare Provider certification required before orientation completion.

Responsibilities

• Coordinate care with clinical partners to achieve discharge outcomes.
• Develop and communicate discharge plans for medically complex cases; document in medical records.
• Facilitate interdisciplinary planning meetings and delegate tasks as needed.
• Utilize post-acute facilities and collaborate with financial partners for safe discharge.
• Provide crisis intervention and psychosocial support for patients and families.
