UNC Lenoir Health Care in Kinston, NC, seeks a Case Manager to coordinate utilization review, discharge planning, and patient education for successful transitions to nursing facilities or community settings.

The role requires nursing or social work background, 3–4 years hospital or community experience, and CN/CSW certification within 5 years. Per diem onsite position with 4 hours weekly.

Formación

• Graduate from an accredited nursing school or as a Medical Social Worker.
• Case Manager Certification to be obtained within 5 years.

Responsabilidades

• Coordinate UR activities and ensure compliance with Federal, State, and third-party guidelines.
• Coordinate discharge planning and patient teaching for transitions to nursing facilities or community re-entry.
• Document psychosocial, financial, and protective services needs; refer as necessary.
• Provide patient and family education and education to staff; support AMD and DNR implementation.
• Collaborate with multidisciplinary team to plan care and ensure smooth transition of care.
• Demonstrate autonomy by prioritizing patient needs and seeking learning opportunities.
