UNC Health in Kinston, NC is seeking a Case Manager to coordinate utilization review activities and discharge planning. The role includes patient teaching, documentation of psychosocial needs, and collaboration with a multidisciplinary team to ensure quality, compliant care and smooth transitions to nursing facilities or community re-entry.

Requires nursing or social work background with case management certification within 5 years.

Formación

• Graduate from an accredited school of nursing or Medical Social Worker.
• Baccalaureate Degree in Human Services area accepted.
• Obtain Case Manager Certification within 5 years.

Responsabilidades

• Monitors utilization review activities for appropriateness of admission and continued stay.
• Coordinates discharge planning between hospital, community agencies, and patients.
• Provides patient/family teaching and education on AMD and DNR when appropriate.
• Documents psychosocial, financial, cultural, domestic violence and protective services needs.
• Serves as a resource for families, staff, community agencies, and physicians.
• Demonstrates autonomy by prioritizing patient needs and seeking professional growth.
