Job summary

Qualifications

• Graduate from an accredited school of nursing or a Medical Social Worker program; Baccalaureate in Human Services accepted.
• Obtain Case Manager Certification within 5 years.
• Three to four years of hospital, nursing home, or community agency experience preferred.
• Licensure/registration as RN or LPN in NC or related social work certification.

Responsibilities

• Coordinate utilization review activities and discharge planning.
• Complete chart reviews and precertifications within policy timelines.
• Document psychosocial, financial, cultural, and protective services needs as required.
• Coordinate with interdisciplinary team for education and patient teaching.
• Serve as a resource for families, staff, and community agencies.
• Promote continuity of care to facilitate transitions to appropriate care settings.
