Qualifications

• Bachelor's degree in Social Work required.
• Three years' experience in medical center, social work or related setting preferred.
• BSW licensure or equivalent licensure (LBSW/LLBSW) required or in process for full licensure.

Responsibilities

• Provide social work intervention related to hospitalization, discharge planning and transitional care needs.
• Collaborate with physicians, nurses and care teams to ensure appropriate level of care and timely transitions.
• Complete and document psychosocial assessments and discharge planning.
• Advocate, mediate, and negotiate with emphasis on patient self-determination.
• Maintain timely, accurate documentation across required systems.
• Lead family meetings and coordinate resources for patients and families.
