Job summary
GBMC HealthCare is seeking a social worker to provide psychosocial assessments, complex discharge planning, and coordination of services for patients. You will collaborate with an interdisciplinary team including RNs, physicians, and residents across units, and assist with distributing patient caseloads to ensure comprehensive care.
Requirements include a Master of Social Work and Maryland licensure (LGSW/LCSW/LCSW-C), with a preference for at least 3 years in a hospital setting.
Qualifications
- Masters in Social Work required.
- Maryland social work licensure (LGSW/LCSW/LCSW-C) required.
- Minimum 3 years clinical social work experience, preferably in a hospital setting.
Responsibilities
- Perform psychosocial assessments and discharge planning.
- Coordinate services, referrals, and support group facilitation.
- Collaborate with RNs, physicians, residents and staff on units.
- Distribute patient caseload and participate in interdisciplinary care planning.
