Qualifications

• Active RN license in the state of Illinois (per ad)
• BLS certification (AHA)
• Strong communication and critical thinking skills
• Ability to work independently in home settings
• 2+ years of recent acute inpatient care or ED experience preferred

• Deliver hospital-level care in patients' homes
• Perform skilled clinical tasks (IV starts, phlebotomy, medication administration)
• Monitor patient conditions and escalate as needed
• Collaborate with virtual and in-person interdisciplinary team
• Coordinate discharge planning and ongoing care needs
• Promote safety and high-quality outcomes
