Job summary

Atrium Health in Charlotte, NC is seeking a qualified nurse or therapy professional to support the Hospital at Home program and coordinate safe post-acute care. You will collaborate with hospital and clinic caregivers, educate patients and families, and assist with discharge planning for post-acute care.

The role emphasizes coordination of transfers, patient needs, and age-appropriate assessments. Licensure as RN, PT, OT, or SLP and at least two years of health care experience are preferred for

Qualifications

• Must be licensed in state the teammate practices – RN, PT, OT or ST License.
• 2 years of health care related experience.
• Strong understanding of Hospital at Home workflows and payors.
• Ability to assess data reflecting patient status and interpret care needs.

Responsibilities

• Collaborates with hospital and clinic caregivers as a resource regarding home care and educates patients/families on services.
• Educates patients/families on home, palliative, and hospice care and assists with discharge planning for post-acute care.
• Attend outcome facilitation team meetings and coordinate with social workers or case managers on complex cases.
• Serve as main communication link between patients, providers, and external partners.
• Coordinate and execute the transfer process to Hospital at Home, including provider notification and consent after medical clearance.
• Ensure timely transfer: notify care team, document acceptance, confirm logistics, and address patient needs (medications, oxygen, equipment).
• Perform patient assessments (skin, cognitive, mobility) and apply clinical judgment in age-appropriate care.
• Demonstrate knowledge to provide care across the lifespan and interpret data to meet patient requirements.
