Job summary

Advocate Health’s Hospital at Home Program in Charlotte, NC, seeks an RN to deliver hospital-level care in patients’ homes, with 12-hour day shifts and travel within the service area. You’ll work with a multidisciplinary team to manage acute needs and ensure safe, effective home-based care.

The role emphasizes autonomy, clinical skill in IV starts and phlebotomy, and strong communication. A driver’s license and IL license compliance are required, with competitive compensation and company vehicle

Qualifications

• Active RN license in the state of Illinois.
• BLS certification (AHA).
• Valid driver’s license with a clean driving record.
• 2+ years of recent acute inpatient care or ED experience preferred.
• Strong IV, phlebotomy, and clinical assessment skills.
• Ability to work independently in home settings.
• Excellent communication and critical thinking.
• Comfortable with technology and remote monitoring tools.

Responsibilities

• Deliver acute, hospital-level care in patients’ homes.
• Perform skilled clinical tasks (IV starts, phlebotomy, medication administration).
• Monitor patient conditions and recognize changes requiring escalation.
• Collaborate with a virtual and in-person interdisciplinary team.
• Use evidence-based practice to ensure safe, patient-centered care.
• Coordinate discharge planning and ongoing care needs.
